Everything in the Adult Plan for anyone under 18 years old whose parent or guardian is an active patient in the office within the last year.
Periodontal Maintenance Plan
Everything in the Adult Plan plus 2 additional cleanings (total of 4 per year, codes listed above, does not include scaling and root planing, D4341, D4342). These four cleaning visits are traditionally called Periodontal Maintenance as opposed to an Adult Prophylaxis.

And after we have our data, we walk you through everything we see and make sure it all makes sense. It’s an important first step in creating a personalized plan that identifies and supports your goals for your long-term health, function, and smile.
**Our Smile Assessment is a two-visit system tailored to give you a complete understanding of your oral health. At your first visit, we gather detailed diagnostic records — including photos, models, and any necessary x-rays — so we can carefully study your case. At your second visit, we meet one-on-one to review the findings, discuss your goals, and create a personalized care plan built around your health, function, and smile.
