Membership plans

Duke Forest Dental Membership Plans make it easy to get the care you need—convenient, transparent visits, high-quality treatment, and no insurance red tape. In fact, for most patients seeking out routine care, our plans may cost less than what you’d pay in premiums and copays with a traditional dental insurance plan.

No annual maximums, no forms, no deductibles, no pre-authorizations, no waiting periods, and no judgment about how often you do or do not actually floss- just straightforward, comprehensive dental care that keeps things simple while keeping you healthy.

Adult plan

  • 2 Checkups (includes an initial “New Patient Introduction*” for any new patient) 
  • 2 Cleanings (D1110, D1120, D4346, D4355, as deemed necessary)
  • 2 Fluoride treatments 
  • 2 Emergency Exams (if necessary) 
  • All exam related xrays (as necessary) 
  • 1 Diagnostic CBCT a year*** 
  • “Smile Assessment**” fee rolled over to cover treatment
  • 2 replacement electric toothbrush heads for the year****
  • 10% off most other UCR fees
  • 5% off retail products 
  • A total UCR (Usual, Customary, and Reasonable) value of $1,385- $1,576

Dependant  plan

(parent/guardian must be an active patient)

Everything in the Adult Plan for anyone under 18 years old whose parent or guardian is an active patient in the office within the last year.

Periodontal Maintenance Plan

Everything in the Adult Plan plus 2 additional cleanings (total of 4 per year, codes listed above, does not include scaling and root planing, D4341, D4342). These four cleaning visits are traditionally called Periodontal Maintenance as opposed to an Adult Prophylaxis.

*Our New Patient Introduction is a one-time, comprehensive evaluation designed to give us a complete understanding of your oral health. During this visit, we collect detailed records — including photos, x-rays, and a thorough clinical exam — so we can assess your health, identify any concerns early, and build a strong foundation for your future care. And after we have our data, we walk you through everything we see and make sure it all makes sense. It’s an important first step in creating a personalized plan that identifies and supports your goals for your long-term health, function, and smile.

**Our Smile Assessment is a two-visit system tailored to give you a complete understanding of your oral health. At your first visit, we gather detailed diagnostic records — including photos, models, and any necessary x-rays — so we can carefully study your case. At your second visit, we meet one-on-one to review the findings, discuss your goals, and create a personalized care plan built around your health, function, and smile.
