What to Bring

We’d love for you to bring a few things along for your first visit.

  • Insurance card (for those with dental insurance)
  • Questions, concerns, and hopes for your smile

Before your visit, please complete your new patient forms that will be sent to you electronically. These can be completed online, or come 15 minutes early to your appointment, and we'll be happy to help you.

We treat you based on your personal needs, not your insurance plan. If you do have dental benefits, we are more than happy to submit claims on your behalf and advocate for you to receive the maximum possible reimbursement. Payment is required at the time of treatment. We offer an in-house membership plan for those without dental insurance.
